Leg extensions for purely cosmetic reasons are not very common. On the other hand, the method is used for medical reasons if a patient can not go properly because one leg is shorter than the other. The operation is usually done as early as possible.

In some countries, this operation is performed using different methods. Among other things, there is the Ilizarov method and this method is an old method which is not recommended and has a lot of complications. An internal nail in the leg is a method but this method has high costs and is wheelchair-bound and bedside throughout the process. Our method is the latest method performed by our Turkish doctor who is one of Turkey’s leading doctors in this area and has only had successful operations. Holyfix is the method we use. This method can also be used if the legs are trapezoidal.
Holyfix applied since 1997 Mono lateral pin type is a method that is an improved version of Wagner and De Bastiani methods and according to the Ilizarov method has fewer complications due to fixtures and better patient compliance.

The nail is inserted in the leg unilaterally outside the Holyfix device. The nails are inserted where there is a lack of blood vessels and nerves, closest to the skin and attached to the safe zone, and the nail stays in the leg and does not extend to the other side. This method prevents the risk of infection. Holyfix is made of special aluminum and titanium alloys that are lighter and MRI compatible. This system is a bio mechanical system that is extremely stable and safe and therefore the patient can be treated. The extension process is very convenient; can be done easily by the patient. Or, I’ll meet the patient at the airport in Istanbul. It takes between 1-2 weeks and we will go back together and I will be available during this time until the Holyfix is removed. We go down first to meet the doctor for inspection and examination and reserve time for surgery.

Holyfix is screwed a little every day. New leg cells will be formed to fill the gap that is constantly created between the bone pieces. The leg was then created to try and heal together. You can extend from the thigh or from the shin or from both shin and thigh if you want to achieve an extension between 10-13 cm. The thigh can be extended between 6-7 cm and the shin 5-6 cm under control depending on the condition of the muscles.¬†With 2 extra operations you can add 5-6 cm. Depending on the doctor’s decision.

The surgery itself is carried out under anesthesia. After surgery, the patient’s legs should be screwed out daily. The extension begins in the hospital. 1 mm extends every day (0.25 mm per 6th hour (4×0.25 mm)) until the specified length is reached. After a few days, the patient is released from the hospital. During this period, our physiotherapist sees that the patient is burden his legs and making sure the patient is walking with crutches with our help and learning and doing muscular exercises, stretching and joining exercises. It is important that the patient do these exercises throughout the process. The patient can shower after the second week. Walking with crutches and cleaning of the nail bottom is made and taught during this time.

Unlike other methods, you can start to walk normally with Holyfix after about 4-6 months depending on how well your muscles are exercised. The holyfix is removed after 9-11 months. Training is the most important part of this period. It’s extremely important to work out, stretch your muscles and go with crutches every day because your muscles do not get stiff and tense when the legs are extended and need to be relieved and adjust the new length.
When it comes to pain, I have to admit that I almost did not feel any pain at all. I myself have undergone this surgery and from my own experience I can say that only pain I felt was muscle ache because the muscles were tense and stiff but you get rid of it by exercising and stretching so there is nothing to complain about if you ask me. I was quite worried about it when I read about it online, but from my own experience, I can say that it really is not so, there is no pain you can not tolerate. Anyway with this method.

There is not a single registered case of leg extension that led to the wheelchair or death. And the legs are even more hardened and durable than before after the bones are healed. As far as I can say, there is nothing to worry about, being patient and doing exactly what the doctor says, everything will work well and without any complications. There may be infection but then you will take antibiotics and it will go over. Infection risk is avoided if you clean the nail bottom thoroughly every other day.

The Holyfix is removed easily and quickly under local anesthesia without pain. Heavy training and sports are allowed after 2-3 months. The intervention leaves small scars where the screws were set after the Holyfix is removed but they are barely visible.
